✅ Before You Apply

Make sure you and your employer have agreed on:

  • A start date and end date
  • Expected number of hours per week
  • Total number of weeks you will be working

How to Complete the I-Plan Application

  1. Access I-Plan

    Go to I-Plan Dashboard and log in using your church login credentials.

  2. Start Your Application

    Click "Internship Approval" from the dashboard.

  3. Create New Request

    Select "Create an Internship Request" to begin a new application.

  4. Complete All Fields

    Fill out all required fields carefully (see guide below for each field).

  5. Submit

    Click "Submit" at the bottom of the form.

Application Fields Explained

📋 Internship Type
Type When to Select
Standard Internship NEARLY ALL internships are this type. Select this if your internship doesn't belong with any of the other types.
Healthcare Facility Only select IF a separate contract requires background check, drug screening, and immunization records. Typically in a hospital setting. Note: Just because it's in a hospital or clinic does not automatically make it this type.
On-Campus For internships taking place on the BYU-Idaho campus.
International Any internships outside of the country. Requires additional documents and approval through the Executive Office. You must submit a detailed explanation and job offer letter.
📅 Internship Semester

Internships must be completed by the end of the semester for which you are registered.

All assignments and hourly reporting will be submitted through the PUBH 498R internship class in I-Learn during the semester you register for.

If your internship is outside semester dates, please check with your internship coordinator.

🏢 Internship Location & Supervisor

Enter the following information about your internship site:

  • Organization name and address
  • Supervisor's full name
  • Supervisor's title/position
  • Supervisor's email address
  • Supervisor's phone number

Your Internship Supervisor is the person at your job site who will be directing your work and evaluating you at the end of your internship. Typically, this is also the person who hired you as an intern.

💼 Job Title

You won't always know what your job title will be. If this is the case, make one up that generally describes your position.

Example:

"Eastern Idaho Public Health Epidemiology Intern"

📝 Job Description

This is where you describe HOW you will be fulfilling the Experience Requirements:

  • Planning, Implementing, or Evaluating a Health/Wellness Program
  • Conducting a Needs Assessment
  • Working with Epidemiology

⚠️ Be Detailed!

The more detail the better. Your application may be denied if you are too vague, and you will be asked to re-submit the application in more detail. It is important that your responsibilities are public health based.

⏰ Hours & Schedule

Total Weekly Hours

On average, how many hours will you be interning each week? This number is agreed upon with your internship supervisor.

To complete the internship hours in one semester, a minimum of 15 hours per week is required to meet the 200 minimum hours.

Start Date / End Date

  • Start date: The first day you begin your internship. Cannot be before your application is approved.
  • End date: Must be on or before the last day of the semester.

Total Anticipated Hours

Must be a minimum of 200 hours.

Formula: Total Anticipated Hours = Weekly Hours × Number of Weeks

This number must align with your weekly hours and the number of weeks indicated by your start and end dates.

💰 Compensation

This is up to your internship supervisor. Most internships will be volunteer/unpaid.

If you are paid, indicate your hourly rate in this field.
